NCLEX Review: Lab Values

It is important for nurses to be knowledgeable about diagnostic lab tests and their values in order to make informed clinical decisions. We will look at several different lab tests in this NCLEX exam review video, including the CBC (Complete Blood Count), HgbA1C, lipid profile, CMP (Comprehensive Metabolic Panel), Coagulation studies (including PT, PTT, and INR), and also ABG’s. We will go over the purpose of these tests and their normal ranges.

Arterial Blood Gas (ABG’s) – Acid/Base Balance

  • pH: 7.35-7.45
  • PaCO2: 35-45 mmHg (always indicates a respiratory issue)
  • HCO3: 22-26 mEq/L (always indicates a metabolic issue)
  • PaO2: 80-100 mmHg
  • SaO2: 95-100%

Comprehensive Metabolic Panel (CMP)

  • Glucose: < 100 mg/dl, 70-99
  • Calcium: 8.5-10.9 mEq/L
  • Albumin: 3.4-5.0 g/dL
  • Total protein: 6.1-8.1 g/dL
  • Sodium (Na+): 135-145 mEq/L
  • Alkaline phosphatase (ALP): 25-100 U/L
  • Alanine aminotransferase (ALT): 7-56 U/L
  • Aspartate aminotransferase (AST): 10-40 U/L
  • Total bilirubin (unconjugated + conjugated): 0.2-1.2 mg/dL
  • Direct bilirubin (conjugated): 0.1-0.4 mg/dL
  • Indirect bilirubin (unconjugated): 0.0-1.1 mg/dL
  • Potassium (K+): 3.5-5.5 mEq/L
  • Chloride (Cl-): 95-105 mEq/L
  • Bicarb (total CO2): 22-26 mEq/L
  • BUN: 8-21 mg/dL
  • Creatinine: 0.8-1.3 mg/dL

Glycosylated Hemoglobin (HgbA1C)

  • Nondiabetic: < 5.7%
  • Prediabietic: 5.7-6.4%
  • Diabetic: ≥ 6.5%

Lipid Profile

  • Total cholesterol: < 200 mg/dL
  • LDL (low-density): < 100 mg/dL
  • HDL (high-density): ≥ 60 mg/dL
  • Triglycerides: < 150 mg/dL

Complete Blood Count (CBC)

  • RBC: 4.5-5.5 mil/uL (males), 4.0-4.9 mil/uL (females)
  • WBC: 4-10.5 thousand/uL
  • Platelets: 140-400 thousand/uL
  • Hgb: 13-17 g/dL (males), 12-15 g/dL (females)
  • Hct: 41-50% (males), 36-44% (females)
